Why the ankle feels sore

The Achilles tendon joins the calf muscles to the heel bone. Although the tendon is not inside the ankle joint, pain from it is felt at the back of the ankle and may spread a short distance upward. Repeated loading, a sudden increase in activity, tight calf muscles or reduced recovery time can irritate the tendon.

Symptoms often build with walking uphill, running, jumping or repeated stair use. The area may feel stiff after rest and loosen somewhat as it warms, then become sore again after activity. Other ankle problems can feel similar, including a sprain or joint irritation, so the history and examination help separate them.

Practical steps

Temporarily reduce running, jumping and hills, but avoid complete inactivity unless a clinician advises it. Choose comfortable shoes and keep walking within a tolerable range. A physiotherapist can prescribe progressive calf and tendon loading; doing too much too soon can flare symptoms. Ice wrapped in cloth may provide short-term comfort.
