What ankle pain can feel like
Ankle pain can come from an injury, tendon problem, joint condition, or instability, so assessment focuses on the symptoms and how they started.
Ankle pain may be felt around the front, back, inner, or outer side of the joint. It can feel aching, sharp, burning, stiff, or tender, and may appear during walking, running, stairs, or at rest. Swelling, bruising, reduced movement, clicking, or a sense that the ankle gives way can add useful clues.
The timing matters: pain after a twist differs from pain that built gradually with activity. Note the location, triggers, swelling, recent footwear or training changes, and whether you can bear weight.
When ankle pain needs prompt attention
Seek urgent medical assessment after a major injury, visible deformity, rapidly increasing swelling, severe pain, numbness, a cold or discoloured foot, or inability to bear weight. Prompt care is also appropriate for fever with a hot, red, swollen joint or drainage from a wound.
If symptoms are mild but do not begin to settle, or if the ankle repeatedly gives way, arrange a clinical review. Urgency is guided by warning signs and function, not by assuming a particular diagnosis.
Possible causes of ankle pain
The following causes are listed alphabetically, not as a likelihood ranking. A clinician uses your history and examination to decide which explanations fit.
[Achilles Tendinitis](/conditions/achilles-tendinitis/) affects the tendon at the back of the ankle and can cause pain with walking or pushing off. [Ankle Fracture](/conditions/ankle-fracture/) can follow a fall, twist, or direct impact. [Ankle Osteoarthritis](/conditions/ankle-osteoarthritis/) may cause stiffness and pain with joint loading. [Ankle Sprain](/conditions/ankle-sprain/) stretches or tears ligaments after the ankle rolls. [Chronic Ankle Instability](/conditions/chronic-ankle-instability/) can cause repeated giving way after earlier injuries. [Peroneal Tendinopathy](/conditions/peroneal-tendinopathy/) affects tendons along the outer ankle.
How ankle pain is diagnosed
A clinician asks how the pain began, where it is located, what movements trigger it, and whether there was an injury. They inspect swelling or bruising, check skin temperature and sensation, test ankle movement and strength, and examine the foot, calf, and walking pattern.
X-rays may be used when a bone injury or joint change is suspected. Ultrasound or magnetic resonance imaging can help assess tendons, ligaments, cartilage, or other soft tissues when the examination and course make that useful. Testing is selected to answer a clinical question rather than to label every ache.
Treatment depends on the cause
Care may begin with activity modification, suitable support, pain relief advice, and exercises that restore movement, strength, and balance. A fracture may require immobilisation and a staged return to weight-bearing; a sprain may need protection followed by rehabilitation. Tendon problems often respond to a gradual loading programme, while arthritis care may combine exercise, footwear changes, symptom relief, and specialist options.
[Sports Physiotherapy](/treatments/physiotherapy-and-rehabilitation/sports-physiotherapy/) can help plan a graded return to activity when sport or repetitive loading is involved. Persistent instability, severe injury, or structural joint damage may need orthopaedic assessment.
Reducing future ankle problems
Build running, jumping, and court-sport loads gradually. Strengthen the calf and muscles that control the ankle, practise balance on stable surfaces, and use footwear that suits the activity. Warm up before demanding movement and replace shoes when their support or tread has clearly deteriorated.
After an ankle injury, complete the rehabilitation plan even when walking feels easier. Regaining balance and confidence can reduce the chance of another giving-way episode. Return to sport in stages, checking that movement and strength tolerate each step.
Symptoms that may occur with ankle pain
Swelling, bruising, stiffness, tenderness, reduced range of motion, weakness, altered walking, or instability may accompany ankle pain. Numbness or tingling can point to nerve involvement, while calf swelling or breathlessness needs urgent assessment because the problem may not be limited to the ankle.
What can happen next
Many episodes improve with appropriate protection and progressive movement. Ongoing pain can limit walking, work, exercise, or sleep, and repeated sprains may leave the ankle less confident on uneven ground. A review can identify whether rehabilitation, imaging, footwear changes, or specialist care is appropriate.

Questions people ask
Should I rest an ankle that hurts?
Reduce movements that sharply increase pain and protect the ankle after an injury, but prolonged complete rest can make stiffness and weakness worse. A clinician or physiotherapist can guide a gradual return to movement.
When should ankle pain be checked?
Arrange assessment for persistent pain, repeated giving way, difficulty bearing weight, or symptoms after a significant injury. Seek urgent care for deformity, a cold or numb foot, severe swelling, or fever with a hot red joint.
