Whooping cough and bronchiectasis

Bronchiectasis involves widened airways that can make it harder to clear mucus and can lead to ongoing cough or phlegm. Whooping cough is a contagious infection that may cause sudden, repeated coughing bouts. The symptoms can overlap, but a new pattern does not establish the cause by itself. In someone with bronchiectasis, a respiratory infection can alter the amount, colour or thickness of phlegm and may worsen breathlessness. A clinician can compare your usual symptoms with the change you have noticed.

Contact your usual respiratory clinician or a GP when your cough is different from baseline, you are bringing up more phlegm, or coughing fits leave you exhausted. Follow any existing airway-clearance plan unless a clinician tells you otherwise, and keep hydrated if you can. Write down when the change started, close contacts with respiratory symptoms, and any fever. Assessment may include listening to your chest and deciding whether tests such as a chest X-ray or spirometry would add useful information.

Get urgent medical help for significant breathing difficulty, blue or grey lips or face, confusion, coughing up blood, chest pain, or a pause in breathing. Seek prompt assessment if you cannot keep fluids down, feel much weaker than usual, or develop fever alongside worsening breathlessness. Babies and frail older adults with severe coughing need early clinical advice.

Ask whether the change suggests an infection, whether your airway-clearance routine should be adjusted, and which warning signs should trigger same-day review. Bring a list of inhalers, antibiotics and other medicines, along with details of your usual phlegm and breathlessness.
