White spots on an infant face

Small pearly bumps can be milia, which are surface-filled cysts that commonly settle without squeezing. Acne-like bumps may also occur in young babies. Heat, saliva, milk, and friction can irritate facial skin and leave tiny bumps or dry pale areas. A smooth light patch, a scaly patch, and a blister have different meanings, so appearance alone should not be used to diagnose the cause.

Infant skin is delicate and absorbs irritation easily. Adult acne products, essential oils, steroid creams, and fragranced cosmetics may worsen a rash unless a healthcare professional specifically recommends them.

Clean the face gently with lukewarm water, pat dry, and keep saliva or milk from sitting on the skin. Use only a bland, fragrance-free moisturiser if dryness is present. Do not pop, scrape, or pick the spots, and avoid powders and medicated creams unless prescribed for the baby. Arrange a routine assessment if the spots persist or the diagnosis is unclear.

Seek same-day medical advice if the baby has fever, seems unusually sleepy, feeds poorly, or the rash is rapidly spreading, hot, swollen, oozing, or blistered. Emergency help is needed for breathing difficulty, facial swelling, or a baby who is difficult to wake.

Ask whether the spots fit milia, irritation, or an infection; whether any product should be stopped; and what changes should prompt review. Note the baby’s age, when the spots began, feeding, temperature, and whether the surface is smooth or scaly.
