White spots on dark skin: possible causes and next steps

Reduced pigment may follow a rash, scratch, burn or other inflammation, sometimes leaving a lighter area after the original problem settles. Dryness can create a pale, flaky surface. Fungal conditions may produce lighter or darker patches with fine scale, especially on the trunk. A condition that reduces pigment production can also begin as a small pale patch and expand. Colour alone cannot identify the cause.

Use a fragrance-free moisturiser, avoid rubbing and pause new products applied directly to the patch. Protect exposed skin from sun because tanning around a lighter area increases contrast and sunburn can worsen irritation. Photograph the patch in consistent light, marking its location and any symptoms such as itch or scale. A clinician can assess colour, texture and borders and select treatment based on the finding.

Arrange an appointment if a patch grows, multiplies, develops scale, becomes itchy or follows repeated inflammation. Seek prompt assessment for rapid change, pain, blistering, bleeding, loss of hair within the patch, or changes involving the eyes, lips or genital skin. Do not use strong bleaching agents or steroid creams without guidance, as they can alter the appearance and irritate darker skin.
