What should you know about white phlegm and lung cancer?

Clear or white mucus can occur when the airways are irritated, inflamed, or clearing an infection. People with lung cancer may have cough or phlegm, but the colour of sputum does not confirm or exclude the condition. Other clues can include a cough that persists or changes, breathlessness, chest discomfort, coughing blood, repeated chest infections, or an unexplained hoarse voice.

A clinician considers smoking or second-hand smoke exposure, workplace irritants, asthma, reflux, infection, medicines, and previous illness. The pattern over time matters more than a single episode.

Book an assessment if phlegm continues, increases, becomes difficult to clear, or appears with a persistent cough or breathlessness. Your clinician may listen to your chest and arrange imaging or other tests. If lung cancer is found, treatment planning can involve staging, tissue analysis, genomic tumour profiling, surgery such as lung lobectomy, radiotherapy, or chemotherapy depending on the individual findings.

Get urgent help if you cough up blood, have severe or rapidly worsening breathlessness, blue or grey lips, sudden chest pain, confusion, or cannot speak in full sentences. New weakness, seizure, severe headache, or a major change in alertness also needs urgent assessment because serious illness can affect more than the lungs.

Ask how long the mucus pattern can be observed, which tests could explain it, and whether your cough or voice change needs imaging. If cancer is suspected, ask how results will guide treatment and whether supportive or palliative care could help symptoms at any stage.
