White Hard Bumps on Skin: What to Know

Small, firm, pearly bumps on the face can be milia, which are tiny keratin-filled cysts near the skin surface. Rough bumps around hair follicles may fit keratosis pilaris, while red or tender follicle-centred bumps can occur with folliculitis. Heat and sweat may contribute to heat rash. Less typical patterns, including flat-topped or violaceous bumps, can have other explanations such as lichen planus.

A clinician considers the body site, colour, itch, pain, scale, drainage, number of lesions, and whether they began after a product or period of sweating. Do not diagnose by colour alone, especially if the bumps are changing.

Wash gently, use non-comedogenic moisturiser, and avoid squeezing, needles, abrasive scrubs, or harsh acids on the bumps. Reduce friction and heat where possible, and pause a new product if irritation started after using it. Treatment depends on the cause: an oral antihistamine may help itch in selected situations, while cryotherapy for lesion removal is reserved for appropriate diagnosed lesions and should not be used on every white bump.

Arrange assessment if bumps persist, multiply, recur, or interfere with sleep or daily activities. Seek prompt care for rapidly spreading redness, warmth, pus, marked pain, fever, facial swelling, or eye involvement. A firm bump that bleeds, crusts, ulcerates, or changes shape also needs medical review.

Ask whether the bumps resemble milia, folliculitis, keratosis pilaris, heat rash, or another condition; which ingredients may help or irritate; whether removal is appropriate; and which symptoms should bring you back sooner.
