White gown hypertension: what to know

Blood pressure naturally changes throughout the day. Feeling tense, talking during a measurement, walking in quickly, pain, caffeine, nicotine, or a full bladder can all influence a clinic reading. The term white gown hypertension does not mean the raised result was imagined; it means the setting may be contributing to it.

A single high clinic reading cannot separate white gown hypertension from sustained hypertension. Checking readings in everyday settings shows whether blood pressure is also raised while you are at home, at work, active, or asleep.

Your clinician may ask you to measure blood pressure at home or wear an ambulatory monitor. Use a validated upper-arm device, sit with your back supported and feet on the floor, keep the arm supported, and avoid conversation while the measurement is taken. Follow the monitoring schedule you are given rather than choosing only readings that seem reassuring.

Bring the device or its stored readings to your appointment. Lifestyle measures that support heart and blood vessel health remain useful, including regular activity, balanced meals, avoiding tobacco, moderating alcohol, and addressing poor sleep. Your clinician will consider whether treatment is needed after reviewing the out-of-clinic pattern and your health history.

Get urgent medical help for chest pain or pressure, sudden weakness, facial drooping, difficulty speaking, severe breathlessness, fainting, or sudden loss of vision. These symptoms need rapid assessment and should not be explained away as anxiety about a blood pressure check.

Arrange a review if your home readings repeatedly exceed the range set by your clinician, you become pregnant, or you start a medicine that may affect blood pressure.

Ask whether home monitoring or a day-and-night monitor is suitable, what cuff size you need, and what results should prompt contact. Tell your clinician about headaches, snoring, daytime sleepiness, kidney problems, and all prescription or non-prescription medicines.
