White frothy sputum and heart failure: what to know

Heart failure can cause fluid to build up in the lungs when the heart is not moving blood forward effectively. Breathlessness, a cough, and white or pink frothy sputum can occur during a sudden worsening. Some people with heart failure also notice ankle swelling, tiredness, or needing extra pillows because lying flat makes breathing harder.

White sputum alone does not establish heart failure. Mucus from airway irritation, infection, reflux, or other lung conditions can also be white and foamy. A clinician considers the full pattern, including breathing, heart symptoms, examination findings, and any known heart condition.

If you already have a heart failure care plan, follow its instructions for new or worsening symptoms and take prescribed medicines as directed. Sit upright if lying flat makes breathing difficult. Do not take extra doses, start a water tablet, or stop heart medicines unless your care plan or clinician tells you to.

Make a note of when the sputum began, whether it is changing, any swelling, palpitations, fever, chest discomfort, and how breathlessness affects usual activity. This information helps a clinician judge whether the problem could be fluid-related or has another cause.

Call emergency services now for severe or rapidly worsening breathlessness, blue or grey lips, confusion, fainting, chest pain or pressure, coughing up pink froth, or inability to speak in full sentences. Do not drive yourself if breathing is severely affected.
