When to worry about whiteheads with comedonal acne

Whiteheads are closed pores containing oil and shed skin cells. They are a very common presentation of Comedonal Acne and often feel like small, firm or smooth bumps rather than tender lesions. Whiteheads can also resemble milia or other bumps, so appearance alone cannot confirm the cause.

Wash gently, avoid scrubbing and do not squeeze or dig at the bumps. Choose non-comedogenic skincare and introduce one acne treatment at a time if a clinician or pharmacist recommends it. Give the routine time to work, and note whether the bumps are spreading, becoming red, or leaving dark marks.

Arrange a dermatology review if the bumps become painful, deep, pus-filled, crusted, or increasingly red; if they affect the eyes; or if repeated picking causes wounds, pigmentation, or scars. Seek prompt medical advice for sudden widespread changes or swelling with breathing difficulty.
