When should you worry about wheezing if you have anaphylaxis?

Wheezing is a common presentation of Anaphylaxis. During this severe allergic reaction, swelling and tightening can affect the airways, making breathing noisy or difficult. Other signs may include swelling of the lips, tongue, or throat; hoarse speech; widespread hives; vomiting; dizziness; or sudden weakness. Skin symptoms do not have to be present for anaphylaxis to be serious.

A wheeze after a likely trigger such as food, an insect sting, or a medicine should be treated as a time-critical warning, especially when symptoms are progressing. Do not wait to see whether the reaction settles on its own.

Use the adrenaline auto-injector prescribed for the person, following its instructions, and call emergency services. Tell the dispatcher that anaphylaxis is suspected. Lay the person down with legs raised if comfortable; allow them to sit if breathing is difficult, and do not let them stand or walk. If symptoms continue and a second prescribed auto-injector is available, follow emergency guidance. Stay with the person and keep monitoring breathing.

Emergency care is needed for any wheezing with suspected anaphylaxis, particularly with throat tightness, voice change, blue lips, faintness, confusion, or collapse. Even if symptoms improve after adrenaline, hospital assessment is required because the reaction can return. Do not drive yourself if breathing is affected; use emergency transport and bring the injector or medicine details if possible.
