When to worry about upper back pain with a vertebral compression fracture

A vertebral compression fracture occurs when a spinal bone becomes compressed, often producing sudden or steadily increasing pain in the mid or upper back. Standing, walking, coughing, or bending may aggravate it, while resting may reduce the load. Most people with Vertebral Compression Fracture notice upper back pain, but pain intensity and location vary. A change in posture or height, tenderness over the spine, or difficulty taking a deep breath can add useful clues for assessment.

Follow the activity and medication advice given by your clinician. Avoid lifting, forceful twisting, repeated forward bending, and high-impact activity until the fracture has been assessed. Move carefully rather than staying in bed for long periods unless specifically instructed. Use support when changing position, and ask about a brace, bone-health assessment, and a safe rehabilitation plan. Do not try to manipulate or straighten the spine yourself.

Contact a healthcare professional promptly if pain is severe, worsening, follows a new fall, or prevents ordinary movement. Seek emergency help for leg weakness, numbness spreading around the trunk or into the legs, difficulty walking, new loss of bladder or bowel control, inability to breathe comfortably, fainting, or fever with a very unwell feeling. These symptoms can signal nerve involvement, another injury, or a problem outside the spine and require direct examination.
