When to worry about cough with bronchiectasis

Cough is a common presentation of Bronchiectasis. The condition can cause mucus to collect in widened airways, making a daily productive cough part of the usual pattern. A sudden increase in cough, thicker or darker sputum, unpleasant-smelling mucus, fever, chest pain, or declining energy can point to an airway infection or flare. Fresh blood should never be dismissed, even if it appears after forceful coughing.

Continue the airway-clearance routine and prescribed medicines unless your clinician has told you otherwise. Note the colour, amount, and thickness of sputum, along with temperature, breathlessness, and chest discomfort. Drink enough fluid for your clinician’s advice, avoid smoke, and keep any sputum sample instructions available. Contact your respiratory team early when your usual pattern changes; they may need to assess you and review treatment.

Call emergency services for severe breathlessness, blue or grey lips, fainting, confusion, or coughing up a large amount of blood. Seek same-day advice for new or increasing blood, fever with worsening sputum, new chest pain, or breathlessness that is greater than usual. Arrange a routine review if cough gradually worsens, sleep becomes disrupted, or airway clearance is becoming harder.
