What to expect from thyroid function test for hyperthyroidism

When hyperthyroidism is suspected, thyroid function testing uses a blood sample to measure signals and hormones produced by the thyroid. The pattern of results is considered alongside symptoms, examination findings, and any medicines or supplements you take.

The thyroid-stimulating hormone result is often assessed with free T4 and sometimes free T3. A low thyroid-stimulating hormone with raised thyroid hormones can support hyperthyroidism, while other patterns may need repeat testing or a different explanation. The blood test does not always identify the cause. Your clinician may consider antibody testing, an ultrasound, or a thyroid scan depending on your history and examination.

Most people do not need to fast. Bring the names and doses of medicines, vitamins, and supplements, and mention biotin, thyroid medicines, recent illness, pregnancy, or a recent change in treatment. A blood sample is collected from a vein and may leave short-lived tenderness or bruising. Take medicines only as instructed; do not alter an antithyroid medicine or thyroid hormone without medical advice. Arrange follow-up to discuss the result and treatment plan.

Seek urgent assessment for chest pain, fainting, severe breathlessness, confusion, or a very fast or irregular heartbeat. Fever with marked weakness, vomiting, agitation, or worsening symptoms while you have untreated or poorly controlled hyperthyroidism also needs prompt care. Do not wait for a routine result if symptoms are severe.
