What causes buttock pain?

Possible sources
Pain on the outer side of the buttock that worsens when lying on that side, climbing stairs, or standing on one leg can occur with gluteal tendinopathy. Pain close to the dimples at the back of the pelvis may involve sacroiliac joint dysfunction, especially after a change in activity or a twisting load. Deep central buttock pain can be associated with piriformis syndrome, where nearby tissues irritate the sciatic nerve. Sciatica may cause buttock pain with tingling, numbness, or pain travelling down the leg. Hip, lower-back, and muscle problems can overlap, so location alone cannot identify the source.
What may help
For a recent flare, reduce the activity that reproduces the pain while keeping comfortable walking and gentle movement. Avoid long periods in one position and change sitting support if that eases pressure. A physiotherapist can assess hip strength, spinal movement, nerve sensitivity, and walking mechanics. Treatment may include graded strengthening, mobility work, and changes to training or work habits. Do not aggressively stretch a painful nerve or return quickly to heavy exercise without guidance.
When to seek care
Seek urgent care for new bladder or bowel difficulty, numbness around the groin, marked leg weakness, or severe pain after major injury. Prompt review is needed for fever, unexplained weight loss, or pain that is constant and worsening. Arrange an assessment when buttock pain persists, repeatedly returns, causes limping, or is accompanied by spreading numbness or weakness.
