Weeping Psoriasis on the Scalp: What to Know

Psoriasis can cause persistent plaques, redness, itching, and thick scale on the scalp. Scratching or lifting scale can create cracks and bleeding. Clear fluid may come from raw skin, while cloudy fluid, crusting, odour, increasing tenderness, or spreading redness can suggest infection or another diagnosis.

Fluid from the scalp does not establish that psoriasis is the cause. Seborrhoeic dermatitis, contact irritation, and infection can look similar, and more than one condition can occur together. A clinician can examine the scalp and hairline and review any recent product or treatment changes.

Do not pick plaques or force scale off. Wash gently with the recommended cleanser, rinse well, and pat rather than rub. Avoid applying hair dye, fragranced products, or home remedies to open or wet areas until a clinician advises you.

Arrange a dermatology review for new weeping, especially if your usual psoriasis treatment is no longer controlling the flare. Bring the names of scalp products and medicines, and describe itching, cracking, pain, and any change in the fluid.

Seek prompt medical care for pus, yellow crusts, foul odour, fever, rapidly spreading redness, marked pain, or tender swelling. Get urgent help if swelling affects the face or eyes, or if you feel seriously unwell.

Ask whether the fluid indicates broken skin, infection, or a different rash; which treatment is safe on open areas; and how to loosen scale without causing bleeding. Ask when to return if the scalp worsens.
