What should you know about very bad ovulation pain?

Ovulation pain, also called mittelschmerz, can cause one-sided lower abdominal discomfort around the middle of a cycle. It may last a short time, but pain that is intense, recurrent, or lasts beyond the expected window can have another explanation. Endometriosis can present with pelvic pain that follows a cycle, while Ovarian Cysts can cause one-sided pain, pressure, or sudden symptoms if a cyst changes.

Pain location alone cannot identify its cause. Pregnancy-related problems, infection, bowel conditions, urinary problems, or a twisted ovary can also produce pelvic pain. A clinician may ask about timing, bleeding, discharge, bowel or urinary symptoms, contraception, and pregnancy possibility before deciding whether an examination or scan is needed.

Record when the pain starts, which side it affects, how long it lasts, and whether it occurs with bleeding, fever, nausea, discharge, or pain during sex. Rest, gentle heat, and a pain medicine that is safe for you may help mild familiar discomfort. Do not rely on self-treatment when pain is severe, keeps returning, or interferes with walking, sleep, or daily activities. Arrange an appointment for recurring episodes.

Seek urgent care for sudden severe pain, faintness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, a swollen or rigid abdomen, or pain with a possible pregnancy. These signs can indicate bleeding, infection, pregnancy complications, or an ovarian emergency. Contact emergency services if you feel faint, confused, or unable to stay safe while travelling for care.
