Vertigo and the eyes: why vision can feel unsettled

You may notice the room bouncing, difficulty focusing, eye flickering or discomfort when turning your head. This can happen because the brain normally combines information from vision, the inner ear and body position sensors. When those messages conflict, the eyes may make small automatic movements called nystagmus. Position-triggered spells can occur with benign paroxysmal positional vertigo. Ear symptoms may point toward an inner-ear problem such as Meniere's disease, while visual sensitivity and headache can accompany migraine. Blurred vision from vertigo is different from sudden loss of sight.

Stop what you are doing and sit somewhere well supported. Fix your gaze on a still, nearby object only if that feels comfortable; do not force your eyes to focus. Reduce fall hazards, avoid bright or rapidly changing screens, and ask someone to accompany you when walking. Rise in stages and do not drive until your vision and balance are reliable. Note whether eye symptoms start with head movement, whether one eye or both seem affected, and whether headache, hearing change or weakness occurs.

Get emergency care for sudden double vision, loss of vision, a new drooping eyelid, unequal pupils, weakness, numbness, difficulty speaking, severe headache, fainting or inability to walk safely. Prompt assessment is also needed for persistent vomiting, new hearing loss, a head injury or visual symptoms that do not settle when the spinning stops.

Ask whether your eye movements suggest a positional cause, whether your vision needs a separate eye examination, and which movements or activities should be limited while symptoms continue.
