What should you know about tryptase angioedema?

Angioedema is swelling in deeper skin or tissue, often affecting the lips, eyelids, tongue, hands, feet, or throat. Tryptase is a substance released by mast cells. A clinician may request a blood sample during or soon after a suspected allergic reaction and compare it with a later baseline result. The result is one part of the assessment; it does not by itself identify every cause of swelling. Angioedema can occur with hives, medicines, foods, insect stings, infections, or inherited and acquired pathways that do not depend on mast cells.

Record when the swelling began, which areas changed, any hives or itching, possible triggers, and medicines taken before it started. Do not stop a prescribed medicine without medical advice, but seek prompt review if an episode follows a new drug. A clinician may discuss an antihistamine treatment when histamine is involved; oral corticosteroid treatment may be considered in selected situations. Avoid known triggers while the cause is being clarified, and keep a dated photograph of visible swelling for review.

Call emergency services immediately for tongue or throat swelling, trouble breathing, wheezing, faintness, confusion, or difficulty swallowing. Rapidly spreading swelling with vomiting or a feeling of collapse also needs emergency assessment. A normal-feeling throat does not make a new severe reaction safe to watch at home. For recurrent swelling without breathing symptoms, arrange medical review so the pattern, medicines, family history, and suitable tests can be considered.

Ask whether the timing of a tryptase sample matters, whether a baseline sample is useful, and which causes fit your pattern. You can also ask whether hives change the evaluation, which medicines should be avoided, and what written emergency plan to follow if swelling reaches your mouth or throat.
