What should you know about tongue cellulitis?

Cellulitis is an infection of deeper soft tissue. On the tongue, inflammation can make the tissue tender, warm, red, and enlarged. A small cut, bite, ulcer, dental problem, or another opening in the mouth can allow bacteria to enter. The swelling may interfere with speaking, chewing, or swallowing. Tongue cellulitis is different from a simple surface irritation because symptoms can progress and may involve nearby tissues.

A clinician may examine the tongue, mouth, teeth, neck, and general condition. Treatment commonly involves prescription antibiotics, with the route chosen according to severity and ability to swallow. Do not squeeze, cut, or apply leftover medication to the area.

Arrange urgent clinical assessment if the tongue is becoming more swollen or painful, or if fever accompanies the change. Until assessed, take small sips of fluid if swallowing is safe, avoid smoking and alcohol, and choose soft foods that do not sting. Follow the prescribed antibiotic schedule exactly if treatment is started, and tell the clinician about allergies, diabetes, immune problems, recent dental work, or medicines.

Call emergency services immediately for trouble breathing, drooling because swallowing is difficult, a muffled voice, rapidly increasing swelling, confusion, faintness, or swelling spreading into the floor of the mouth or neck. These signs can indicate that the airway is at risk or that a severe infection is developing. Fever with marked weakness also needs urgent evaluation because serious infection can spread beyond the original area.

Ask what may have allowed the infection to start, whether a tooth or mouth wound needs attention, how quickly improvement should begin after treatment, and which symptoms mean you should return immediately. Clarify how to take each medicine and whether you need a follow-up examination.
