TOLAC after two C-sections: what to know

TOLAC means trial of labour after caesarean. The aim is a vaginal birth, while recognising that labour may end with another caesarean. After two operations, your clinician will review the type and position of each uterine incision, whether the surgical and recovery courses were uncomplicated, your current pregnancy, the position of the placenta and the baby's position. A previous classical or other high uterine incision may change the options.

A planned repeat caesarean may be advised when the expected risks of labour are too high or when vaginal birth is unsuitable for another reason. The decision is personal and should include your preferences, the local hospital's emergency facilities and the experience of the maternity team.

Ask for a review of your previous operation notes early in pregnancy. Discuss how labour would be monitored, what signs would prompt a caesarean, pain relief choices and how induction would affect the plan. Spontaneous labour and induction are not interchangeable; methods and timing need specialist selection after a uterine scar.

Bring a clear birth preference document, but keep it flexible. Arrange maternity care where urgent surgery, anaesthesia, blood testing and newborn support are available. If a repeat caesarean is chosen, ask about recovery, movement and pelvic-floor support; postnatal physiotherapy may help with safe rehabilitation.

Contact your maternity unit urgently for regular painful contractions, vaginal bleeding, leaking fluid, reduced fetal movement, severe constant abdominal pain or feeling faint. Do not wait for a routine appointment when these symptoms occur after previous uterine surgery. The team can assess you and decide whether labour is beginning or another problem needs attention.
