What should you know about tiny red freckles on skin?

Classic freckles are flat tan-to-brown spots that often become more visible after sun exposure. A dot that is bright red, purple-red, raised, or prone to bleeding may not be a freckle. Colour can also look different across skin tones and under indoor lighting, so visual description alone cannot identify a lesion.

Record whether each mark is flat, whether it changes with sun, and whether its border, colour, size, or surface changes. A clinician can inspect it with magnification and decide whether observation or further assessment is appropriate.

Use broad-spectrum sun protection on exposed skin, seek shade, and avoid deliberate tanning. Do not pick, scratch, or use strong lightening products on an unassessed red mark. Take periodic photographs in consistent lighting and note any new symptoms such as crusting, bleeding, tenderness, or rapid change.
