What may be happening

Ovulation pain, also called mittelschmerz, can occur on one side of the lower abdomen around the middle of a cycle. It may feel like an ache, cramp, or sharp twinge and can shift sides between cycles. Mild discomfort can settle without treatment, but terrible pain is not explained by ovulation alone without considering other causes.

Endometriosis and Ovarian Cysts can cause pelvic pain that overlaps with mid-cycle symptoms. The timing of pain is useful information, but it cannot identify a diagnosis by itself. A clinician may ask about bleeding, discharge, fever, bowel or bladder changes, sex-related pain, and pregnancy possibility.

Record when the pain begins, which side hurts, how long it lasts, and whether it follows a cycle pattern. Note bleeding, fever, nausea, discharge, or pain during movement. Make an appointment if pain keeps returning, is severe enough to limit normal activities, or does not settle as expected. Bring your symptom record and mention any contraception or fertility plans, since these details can guide the assessment.

Red flags

Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, repeated vomiting, a rigid or very tender abdomen, or pain when pregnancy is possible. Do not wait for the next cycle if symptoms are escalating. Prompt assessment can distinguish a gynaecological emergency from a recurring but non-urgent pattern.
