Understanding the pain

The iliotibial band is a strong strip of connective tissue running along the outside of the thigh. Irritation can develop when running, cycling, hill work, or a rapid training change repeatedly loads the outer knee or hip. People may notice a sharp or burning ache during repeated knee bending, often at a predictable point in a run. Tenderness near the outer knee does not by itself confirm the diagnosis; hip strength, foot mechanics, the knee joint, and training habits also need consideration.

Helpful first steps

Reduce hills, speed work, long rides, or other movements that bring on the pain, while retaining low-irritation activity if it is comfortable. Do not use stretching or foam rolling as a reason to push through sharp symptoms. Physiotherapy can assess running or cycling technique and prescribe progressive hip and leg strengthening. Strength training rehabilitation helps the tissues tolerate demand again. Shockwave therapy or PRP injection may be considered in selected persistent cases after assessment.

When to seek care

Get urgent assessment if the knee becomes hot and very swollen, you cannot bear weight, the leg looks deformed, or pain follows a significant fall or twist. Arrange a routine review when outer-knee pain limits walking, recurs each time you exercise, or fails to settle after activity modification. Numbness, weakness, locking, or pain at rest also warrants a broader examination.
