What should you know about temperature after seizure?

A raised temperature after a seizure can be temporary, but persistent fever or other warning signs need prompt medical assessment. Muscle activity during a convulsive seizure can leave the body warm and sweaty for a while. Fever can also be present before a seizure or result from an infection that needs treatment, so the timing and the person's recovery both matter.

Check whether the person is becoming more alert, breathing normally and returning to their usual behaviour. A warm skin or flushed face is not enough to confirm fever; use a reliable thermometer when it is safe to do so. A temperature that settles as recovery progresses may reflect exertion, clothing or a warm room.

A continuing or high fever may be unrelated to the seizure, may have contributed to it, or may signal an infection. Fever with severe headache, a stiff neck, a new rash, marked drowsiness or unusual confusion is particularly concerning because serious infections such as meningitis can affect the brain and its coverings. Other possible triggers include low blood sugar or withdrawal from heavy alcohol use; these need medical evaluation rather than home treatment.

Stay with the person and place them on their side if they are sleepy or not fully responsive, keeping the airway clear. Remove extra blankets and move them away from heat, but do not use ice or cold-water immersion. Once they are fully awake and swallowing normally, offer small sips of water. Do not give food, drink or tablets while they are drowsy, and do not put anything in their mouth.

Record the temperature, how it was measured, when the seizure ended and any symptoms that followed. Avoid giving fever medicine unless a clinician has advised that it is suitable for the person, especially if they remain confused, are vomiting or have another medical condition. Arrange medical advice for a first seizure, a new fever, or a temperature that does not settle during recovery.

Call emergency services if the seizure is still happening after five minutes, another seizure starts before full recovery, the person has trouble breathing, remains unresponsive, has a serious injury, or the seizure occurred in water. Seek urgent assessment for fever with a stiff neck, severe headache, a spreading rash, repeated vomiting, worsening confusion or unusual difficulty waking.

Do not drive the person yourself if they are not fully alert or are having another seizure. Keep the area safe, note the seizure's start and end time, and follow the emergency operator's instructions. A person who has diabetes, may have stopped heavy alcohol use, is pregnant, or has a weakened immune system should receive prompt clinical advice after a seizure even if the temperature later improves.

Ask what may have triggered the seizure, whether the temperature needs monitoring at home, and which symptoms should prompt emergency help. Tell the clinician about recent illness, medicines, alcohol use, diabetes, missed meals, head injury and any unusual movements or behaviour witnessed during the event.
