What is the TBIL blood test?

Bilirubin is produced as the body processes older red blood cells. The liver changes it so it can leave the body in bile. A total bilirubin result combines forms of bilirubin and is usually considered alongside other liver tests and the clinical history.

An altered result has several possible explanations. It can relate to red-cell breakdown, liver inflammation, reduced bile flow or an inherited difference in bilirubin handling. Conditions such as Anaemia, Brucellosis, glandular fever or digestive disease may be considered only when the symptoms and other findings fit; a single result does not identify a cause.

Check the laboratory report for the full test name, units and reference range, then discuss it with the ordering clinician. Mention yellowing of the skin or eyes, dark urine, pale stools, abdominal discomfort, itching, fever, recent illness, alcohol use and medicines or supplements. Do not stop prescribed treatment solely because of one abnormal value.

Further blood tests or an examination may be needed. The clinician may also consider related conditions such as chronic kidney disease, coeliac disease, Crohn's disease or dengue fever when the wider pattern points in that direction.

Arrange prompt review for new yellow skin or eyes, very dark urine, pale stools, persistent vomiting, worsening abdominal pain or fever. Seek urgent help for confusion, fainting, severe pain or rapidly worsening illness. These symptoms need assessment rather than interpretation from the report alone.
