How position can affect symptoms

Orthostatic hypotension is a drop in blood pressure when a person moves from lying or sitting to standing. The body normally tightens blood vessels and adjusts the heart rate to keep blood reaching the brain. If that response is delayed or insufficient, dizziness, blurred vision, weakness, or a faint can follow. Dehydration, vomiting, diarrhoea, heat, prolonged bed rest, and some medicines can contribute. Diabetes, nerve disorders, or heart conditions may also affect the response. Symptoms are often strongest in the morning or after a meal, but the pattern needs individual assessment.

Reducing the chance of a fall

Pause at the edge of the bed before standing, then rise in stages while holding a stable surface. Flexing the ankles and tightening the leg muscles before standing may help some people. Drink fluids regularly unless a clinician has given you a fluid restriction, and avoid overheating. Sit or lie down as soon as dizziness begins. Keep pathways clear, use good lighting, and do not change prescription medicines or add salt without clinical advice. A GP consultation can review symptoms, medicines, blood pressure in different positions, and possible causes.

When assessment is urgent

Get emergency help for fainting with chest pain, severe breathlessness, a rapid or irregular heartbeat, major injury, abnormal breathing, persistent confusion, new weakness, or difficulty speaking. Urgent assessment is also needed when collapse occurs during exercise, while lying down, or without warning. Arrange prompt review if episodes recur, falls are becoming a risk, or dizziness continues despite sitting or lying down. Bring a list of medicines and describe the exact position change that preceded symptoms.

Is dizziness on standing the same as syncope?

No. Dizziness may occur without losing consciousness; syncope means a brief loss of consciousness. Both can arise from a postural blood-pressure drop.

Should I stop my blood-pressure medicine?

Do not stop it without medical advice. A clinician can check whether the dose, timing, or another medicine needs review.
