Swollen ankles and heart failure: what to know

When the heart does not move blood effectively, pressure can rise in nearby veins. Fluid may then pass into tissues around the ankles, especially after standing or sitting. Swelling in both ankles, breathlessness, unusual tiredness, waking short of breath, or a rapid change in body weight can make heart failure more concerning. Ankle swelling alone does not identify its cause. Chronic kidney disease, obesity, varicose veins, medicines, and prolonged inactivity can also contribute.

Arrange a medical assessment if the swelling is new, persistent, or increasing. Note whether both ankles are affected, when swelling is worst, and whether shoes or rings feel tighter. Rest with your legs raised when comfortable, avoid sitting still for long periods, and follow any fluid, salt, or medicine plan already prescribed. Seek emergency help for severe or rapidly worsening breathlessness, chest pain, fainting, confusion, or blue-grey lips. Contact a clinician promptly if swelling suddenly worsens, one leg becomes painful or red, or you develop breathlessness when lying down. These features need assessment because they may signal a circulation problem, a clot, or worsening heart function.
