What should you know about strong ovulation pain?

Ovulation pain, also called mittelschmerz, can feel like a cramp, twinge, or sharp ache on one side of the lower abdomen. It may occur around the middle of the cycle and settle as the ovary and nearby tissues stop reacting. Pain that lasts longer, returns outside the mid-cycle pattern, or interferes with normal activity is less typical of simple ovulation pain. Endometriosis and ovarian cysts can also cause pelvic pain, so the timing alone cannot identify the cause.

Note when the pain begins, which side it affects, how long it lasts, and whether bleeding, discharge, nausea, fever, or urinary symptoms occur with it. Rest, gentle heat over the lower abdomen, and fluids may help mild discomfort. Use an over-the-counter pain medicine only if it is suitable for you and follow the label; check with a clinician or pharmacist if you are pregnant, have stomach or kidney problems, or take other medicines. Arrange a review if episodes are becoming stronger or affecting sex, work, sleep, or movement.

Seek urgent medical help for sudden severe pain, fainting, marked weakness, heavy vaginal bleeding, fever, repeated vomiting, a swollen abdomen, or pain during a possible pregnancy. Contact a clinician promptly for pain that does not ease, keeps returning, or comes with unusual discharge or pain when urinating. These warning signs can occur with conditions that need treatment and should not be explained away as ovulation.
