What should you know about steeple sign croup?

The steeple sign is a narrowing seen near the voice box on a neck or airway image. In the right clinical setting, it can support a diagnosis of croup, an illness that commonly follows a viral upper-respiratory infection. The narrowing reflects inflammation around the upper airway rather than a diagnosis that can be judged from an image alone.

Croup often causes a harsh, barking cough and a raspy or high-pitched sound when breathing in. A cough may occur with or without fever. The sound is called stridor. It can become more noticeable when a child is upset, crying, or active, because these situations increase airflow demand.

Keep the child calm and upright, offer small sips of fluid if they can swallow comfortably, and watch their breathing rather than relying on the cough alone. Avoid forcing food or drink during a breathing struggle. Do not use someone else’s prescription medicine or start antibiotics without clinical advice.

A clinician may assess the child’s breathing, oxygen level, hydration, and ability to speak or cry. Treatment can include medicine to reduce airway swelling; more severe symptoms may need monitored urgent care and inhaled treatment such as a nebuliser. Seek emergency help immediately if the child is struggling to breathe, the skin between the ribs or at the neck pulls inward, the lips look blue or grey, the child is unusually sleepy or difficult to wake, or they cannot speak, cry, or swallow because of breathlessness. If the cough or noisy breathing is worsening, contact a clinician promptly even if the child settles between episodes. Keep observing after an initial improvement because airway swelling can change, particularly overnight.

Ask what the image shows, whether the breathing noise is present at rest, which treatment is appropriate, and what signs should trigger emergency care. Tell the clinician when symptoms began, whether there has been fever, and whether the child is drinking and urinating normally.
