What should you know about stage four bladder cancer?

Doctors use scans, tissue results and examination findings to understand where the cancer is located. It may involve nearby organs or lymph nodes, or it may have travelled to a more distant part of the body. The exact pattern of spread, the tumour type, previous treatment and your general health all affect the plan. A cystoscopy may help examine the bladder lining, while imaging helps map disease outside it.

Stage four is advanced disease, but it does not describe one identical situation for every person. Your oncology team can explain which areas are involved and what the treatment is intended to achieve.

Treatment may include medicines that travel through the bloodstream, such as chemotherapy or immunotherapy. Surgery to remove the bladder is considered only in selected circumstances, and local treatment may be used for symptom control. Intravesical BCG is generally used for particular bladder cancers contained within the bladder, so ask why it is or is not suitable for your disease.

Contact your cancer team promptly for inability to pass urine, worsening blood in the urine, fever, severe pain, new confusion, marked weakness or repeated vomiting. Difficulty passing urine can require urgent assessment because blockage may affect the kidneys. Seek emergency help for heavy bleeding, collapse, severe breathlessness or sudden worsening symptoms.
