Spotting Before a Miscarriage: What to Know

Miscarriage is a pregnancy loss, and bleeding may be one of its signs. However, spotting can also happen in a continuing pregnancy. Changes that raise concern include bleeding that becomes heavier, cramps that intensify, passage of tissue, or a reduction in pregnancy symptoms, but symptoms alone cannot establish what has happened.

Assessment may include questions about the bleeding and pain, a pregnancy hormone blood test, and a Transvaginal Ultrasound. Sometimes the first assessment cannot give a definite answer, so repeat testing or review is arranged. This uncertainty can be emotionally difficult; ask for clear instructions about follow-up.

Use pads, record the amount and colour of blood, and note cramps, clots, or tissue. Contact your pregnancy-care team promptly. If you know your blood group, share it with the clinician. Avoid tampons and do not self-treat with medicines unless a healthcare professional has advised that they are suitable.

Seek emergency care for heavy bleeding, severe or one-sided pain, shoulder-tip pain, fainting, severe dizziness, fever, or feeling weak or unwell. These symptoms can indicate significant blood loss, infection, or ectopic pregnancy. Take urgent action even if a previous scan or test was reassuring.

Ask what the scan or blood-test result shows, whether repeat assessment is needed, how to recognise infection, and which service to contact overnight. If loss is confirmed, ask about each management option, pain relief, emotional support, and when follow-up will occur.
