What should you know about spasmodic croup treatment?

Croup is swelling around the voice box and upper airway. A spasmodic episode can begin suddenly, commonly during the night, with a harsh barking cough, hoarse voice, or a high-pitched sound when breathing in. A cold may precede the episode, but the cough can appear after the child seemed well. Crying and distress can make the noisy breathing worse.

A clinician assesses the child’s breathing, colour, alertness, ability to drink, and the sound heard at rest. Mild symptoms may settle with calm observation, while persistent noisy breathing or visible effort needs medical assessment. Croup can change over a short period, especially in younger children.

Hold the child upright and use a calm voice. Offer small sips of fluid if swallowing is comfortable, and avoid forcing food or drink during a breathing struggle. Keep smoke, strong fragrances, and unnecessary handling away from the child. Do not use steam or give cough medicines unless a clinician has advised them.

A clinician may prescribe a steroid medicine to reduce airway swelling. More severe symptoms may need urgent observation and nebuliser treatment. A child health consultation can help assess repeated episodes, fever, hydration, and other possible causes.

Seek emergency help if the child is struggling to breathe, the skin between the ribs is pulling in, the lips look blue or grey, the child is unusually sleepy, or breathing pauses occur. Urgent assessment is also needed if noisy breathing continues while the child is resting, drooling prevents swallowing, or the child cannot speak or cry normally because of breathlessness.

Ask what signs show that the episode is mild or worsening, whether prescribed medicine should be kept at home, how to handle a night-time recurrence, and whether another illness could be causing the breathing noise.
