Sore Itchy Flaky Scalp: Causes and Care

Fine white flakes may occur with a dry or dandruff-prone scalp. Yellowish greasy scale around the scalp, hairline, ears, or eyebrows is more suggestive of seborrheic dermatitis. Thick silvery plaques can occur with scalp psoriasis. Broken hairs, tender areas, or a spreading scaly patch need assessment for scalp ringworm, especially in children.

Use lukewarm water and a gentle cleanser, and loosen flakes without scraping. Avoid fragranced styling products and harsh scratching while the skin is sore. A clinician may recommend medicated shampoo therapy or another treatment based on whether yeast-related inflammation, psoriasis, or infection is present. Follow the product directions and stop if burning or a new rash develops.

Book an appointment when flaking is persistent, painful, spreading beyond the scalp, or associated with broken hairs or hair loss. Seek prompt care if the scalp becomes hot and swollen, produces pus, or you develop fever. Do not share combs, brushes, or hats while an infectious cause is being considered.

Ask what the scale looks like under examination, whether the hair shafts are affected, and how long to use a recommended shampoo. Tell the clinician about eczema, psoriasis, recent travel, pets, household symptoms, and any new hair products.
