Sore breasts and an itchy vulva

Sore breasts and an itchy vulva may be separate symptoms, so consider hormonal, irritant, infectious, and pregnancy-related causes. Breast tenderness can follow hormone changes, while vulval itching may come from soaps, pads, shaving, dermatitis, thrush, or another infection. The combination does not identify one diagnosis.

Hormonal changes can make breasts tender and alter vulval moisture or sensitivity. Contact irritation is more likely when itching began after a new wash, detergent, pad, liner, lubricant, or hair-removal product. Thrush may cause intense itching with soreness or thick discharge, while other infections can cause odour, unusual discharge, pain when urinating, or pelvic discomfort. Breast redness, warmth, swelling, fever, or nipple discharge needs separate breast assessment.

Stop fragranced products on the vulva, wash gently with lukewarm water, pat dry, and choose loose breathable underwear. Avoid scratching, douching, and using leftover steroid or antifungal creams without advice because they can mask the cause. A supportive bra and a cool compress may ease breast soreness. If pregnancy is possible, test when appropriate and ask a pharmacist or clinician before taking medicines. Avoid sex until pain or discharge has been assessed if an infection may be possible.

Arrange a review if itching lasts, returns, causes broken skin, or comes with discharge, sores, bleeding, urinary pain, or pelvic pain. Seek prompt breast assessment for a new lump, bloody discharge, nipple inversion, dimpling, redness, fever, or rapidly increasing swelling. Urgent help is needed for severe pelvic pain, fainting, facial swelling, or difficulty breathing.

Ask whether the vulval skin needs examination or a swab, whether a medicine or product is irritating it, and whether breast examination is needed. Mention pregnancy possibility, recent antibiotics, new products, discharge, sexual health concerns, and any breast changes.
