What should you know about smooth goitre?

The thyroid sits at the front of the neck and helps regulate how the body uses energy. With a smooth goitre, the whole gland feels or looks enlarged rather than having a clearly separate lump. The swelling can be small or more noticeable, and thyroid activity can remain normal even when the gland is enlarged.

A goitre can occur alongside hyperthyroidism, hypothyroidism, or neither. The appearance alone cannot identify the cause. An examination and blood tests help show whether the thyroid is making the right amount of hormone; the clinician may arrange further assessment if the swelling, symptoms, or examination findings warrant it.

Arrange a GP consultation if you notice new fullness at the front of your neck, a change in the way a collar fits, or symptoms such as palpitations, tremor, heat intolerance, tiredness, feeling unusually cold, or unexplained weight change. Take a list of medicines and supplements, and mention any previous thyroid problems or family history.

Assessment commonly includes feeling the thyroid and checking nearby areas of the neck. A thyroid function test can help determine whether hormone production is low, high, or within the expected range. Do not start iodine or thyroid supplements without medical advice, because the appropriate treatment depends on the cause and your hormone levels.

Seek urgent medical help if neck swelling makes breathing difficult, causes choking, or makes swallowing liquids hard. Get prompt assessment for a rapidly enlarging swelling, a new persistent hoarse voice, or significant neck pain. These features do not identify a diagnosis by themselves, but they should not be left unexplained.

If breathing is severely restricted or you cannot swallow, use emergency services. Otherwise, arrange a timely appointment so the clinician can examine the swelling and decide whether blood tests or other investigations are needed.
