How smoking affects reflux

The lower oesophageal sphincter normally helps prevent stomach contents moving upwards. Smoking can interfere with this barrier and irritate the oesophagus, while coughing linked with smoke exposure can add pressure through the abdomen. Reflux may feel like burning behind the breastbone, a sour taste, throat irritation, or a cough that is worse after meals or when lying down.

Symptoms can have more than one contributor. A hiatus hernia, excess body weight, certain foods, or medicines may also affect reflux. Chest tightness is not automatically reflux, so new or concerning chest symptoms should be assessed rather than self-treated.

Stopping smoking removes a trigger and supports healing of irritated tissues. If quitting feels difficult, ask a healthcare professional about a structured plan, nicotine replacement, or prescription options. Smaller meals, avoiding lying down soon after eating, and raising the head of the bed can reduce night-time backflow for some people.

Notice whether alcohol, large meals, spicy foods, or late eating trigger symptoms, but do not remove many foods unnecessarily. A pharmacist can advise whether an antacid or acid-suppressing medicine is suitable, including how long to use it and when a review is needed.

When to seek care

Get urgent help for severe or persistent chest pain, fainting, marked breathlessness, vomiting blood, or black stools. Arrange a medical review for food sticking, painful swallowing, repeated vomiting, unexplained weight loss, or symptoms that keep returning despite sensible changes.

A persistent cough or sleep disruption may need assessment for reflux and other causes. A clinician can distinguish reflux from heart or breathing problems and choose treatment safely.
