Smile Lines Under the Eyes: What to Know

The skin beneath the eyes is thin and moves whenever you smile, squint or narrow your eyes. Fine lines may become more visible when the area is dry, after repeated sun exposure, or when natural facial volume changes the way light falls across the skin. A line beneath the eye is separate from a crease at the outer corner, although both can be expression-related.

Persistent puffiness, itching, scaling, burning or tenderness suggests more than a simple line. New asymmetry can also change the appearance of one side and merits a closer look.

Use a bland moisturiser suitable for the eye area, avoid rubbing, and wear sunglasses with facial sun protection outdoors. Introduce new products cautiously because fragrance and active ingredients can irritate thin skin. If contact lenses, cosmetics or allergies seem to trigger symptoms, mention that pattern during an appointment.

A clinician can assess whether texture, movement or volume is the main concern before discussing procedures. Injectable treatments around the eyes require specialised knowledge; a thread lift or volume treatment is not automatically appropriate for a line in this location.

Seek urgent care for eye pain, vision changes, severe swelling, facial weakness or breathing difficulty. Arrange prompt review for a hot, red, painful eyelid, discharge, a spreading rash or a new one-sided change. Lines that are stable and symptom-free can usually be discussed at a routine cosmetic or dermatology consultation.

Ask whether the line is under the eye or at its outer corner, what may be driving it, and which non-procedure measures are worth trying. If treatment is discussed, ask about eye-area risks, recovery, alternatives and the clinician's experience with this anatomy.
