Small red spots on the penis: what to know

Notice whether the spots are on hair-bearing skin, the shaft, or the head; whether they are smooth, raised, itchy, sore, scaly, or filled with fluid; and whether they appeared after friction, shaving, a new product, or sexual contact. Folliculitis can affect hair follicles and may look like tender red bumps. Irritant dermatitis can follow soaps, lubricants, or prolonged rubbing. A spot that becomes an ulcer, blister, crust, or discharge needs a clinician's assessment because different infections can look alike.

Wash gently with lukewarm water, pat dry, and pause fragranced products and vigorous scrubbing. Keep the area dry and wear breathable underwear. Do not squeeze, pierce, or apply steroid, antibiotic, or antifungal cream without advice, especially on the penile tip. Avoid sexual contact until painful or unexplained lesions have been assessed; use clear communication with partners if an infection is possible. A clinician may inspect the skin and arrange testing based on the pattern.

Arrange prompt medical care for spreading redness, marked swelling, severe pain, fever, pus, difficulty passing urine, blisters, open sores, or a new rash after sexual exposure. Seek urgent help if swelling rapidly affects the foreskin or urination. Spots that do not settle after removing an irritant, repeatedly return, bleed, or change in colour or shape also deserve a dermatology or sexual-health review.
