Small pea-sized lumps in the neck: what to know

Neck lymph nodes can become noticeable as the immune system responds to infection in the throat, ears, teeth, scalp, or skin. More than one small node may be felt because drainage areas overlap. Cysts, salivary glands, thyroid tissue, and normal anatomical structures can also be mistaken for nodes. Persistent or changing lumps may lead a clinician to consider head and neck cancer, lymphoma, thyroid cancer, or cancer of unknown primary, but size and touch cannot make that diagnosis.

Write down how many areas you feel, where they are, when they appeared, and whether they are tender or changing. Check for recent infection, mouth or dental symptoms, skin irritation, fever, voice change, or swallowing trouble. Avoid repeatedly prodding the neck, which can make it sore and harder to judge. Arrange clinical review if the lumps persist, enlarge, become hard or fixed, or have no clear recent trigger. Examination may be followed by imaging or sampling when indicated.

Seek urgent help for difficulty breathing or swallowing, sudden swelling, or severe illness. Seek prompt medical review for persistent lumps, progressive enlargement, unexplained hoarseness, bleeding, ongoing pain, night sweats, fever, or unexplained weight loss. These symptoms guide urgency; they do not mean that cancer is present.

Ask whether the areas feel like lymph nodes or another structure, whether an infection or dental issue could account for them, and what change should trigger reassessment. Ask whether ultrasound or a tissue sample is needed. If the thyroid is involved, ask how thyroid removal surgery would be considered.
