What should you know about a small lump behind the neck?

Lymph nodes behind the neck can enlarge during infections affecting the scalp, skin, ears, or throat. A blocked follicle, cyst, or local inflammation can create a lump in or just beneath the skin. A deeper, firm, or persistent mass needs a clinician to identify its source. Head and neck cancer and lymphoma can present with a neck lump, while cancer of unknown primary is considered when a metastatic deposit is found without an obvious starting site. These conditions cannot be distinguished by touch alone.

Do not squeeze or repeatedly press the lump. Check gently for changes in size, mobility, tenderness, redness, or drainage, and note recent infections, scalp irritation, dental symptoms, or skin breaks. Book an examination if it remains, enlarges, or feels hard or fixed. The clinician may inspect the scalp, ears, mouth, and throat, feel other lymph-node areas, and arrange imaging or tissue sampling when appropriate. The test chosen depends on the lump’s depth and examination findings.

Seek prompt care for a rapidly growing lump, spreading redness, pus, fever, severe pain, or a hard fixed mass. Arrange timely assessment for night sweats, unexplained weight change, persistent fatigue, voice change, swallowing difficulty, or a lump that does not settle after an illness. Get urgent help for breathing difficulty, severe weakness, or sudden neurological symptoms.

Ask whether the lump is likely to be in the skin or a lymph node, whether an ear, scalp, dental, or throat source is present, and when imaging is useful. Ask what changes should prompt review sooner and whether other neck areas need examination.
