Sinus drainage cough: what should you know?

Sinus drainage can cause coughing when mucus flows down the throat and irritates sensitive airway tissues. The cough may be worse when lying down, after waking, or when the nose is blocked. The sound of the cough does not identify its cause by itself.

A cold can increase mucus for several days. Allergic rhinitis may cause repeated drainage, sneezing, itching, and a blocked nose. Acid reflux can also irritate the throat and create a cough that feels similar to post-nasal drainage. A cough with wheezing, breathlessness, or repeated chest infections needs a broader assessment; asthma, acute bronchitis, or bronchiectasis can involve cough for different reasons.

Pay attention to whether the mucus is clear or coloured, whether you have fever, and whether symptoms are improving or continuing. Thick mucus alone does not prove that antibiotics are needed.

Drink fluids regularly, use a saline nasal spray or rinse as directed, and avoid smoke and strong fragrances. Sleeping with your upper body slightly raised may reduce drainage-related coughing at night. Honey may soothe a cough for adults and children over one year; never give honey to an infant under one year. Seek urgent help for severe breathing difficulty, blue or grey lips, chest pain, confusion, or coughing blood. Arrange medical review if the cough is worsening, disrupts sleep repeatedly, follows a choking episode, or comes with persistent fever, wheezing, or breathlessness. Children, older adults, and people with lung disease may need earlier assessment.
