Silent Acid Reflux: Symptoms and Next Steps

Reflux happens when stomach contents move upwards into the oesophagus and sometimes towards the throat. Some people notice a persistent cough, frequent throat clearing, a hoarse voice, a sour taste, a lump sensation, or difficulty sleeping rather than burning behind the breastbone. These symptoms are not specific to reflux.

Acid reflux may be more likely with a hiatus hernia, certain meals, lying down soon after eating, or increased abdominal pressure. Obesity can contribute in some people, but body size alone does not establish the diagnosis. Chest tightness or abdominal pain can have causes that need separate assessment.

Keep a brief symptom diary covering meals, timing, posture, cough, voice changes, and sleep. Try smaller evening meals, avoid lying down soon after eating, and raise the head end of the bed if night symptoms are prominent. Notice personal triggers rather than removing many foods at once. A clinician or pharmacist can advise whether a reflux medicine is appropriate and how long to try it.

Arrange a medical review for persistent cough, repeated hoarseness, painful or difficult swallowing, unexplained weight loss, vomiting, or symptoms that keep disturbing sleep. Seek urgent care for severe or crushing chest pain, breathlessness, fainting, vomiting blood, or black stools. Chest symptoms should not be assumed to be reflux, particularly when they start suddenly or occur with sweating or weakness.
