Shoulder joint pain and weakness

Pain after lifting or a fall may affect the rotator cuff, joint, or surrounding muscles. Nerve irritation can cause weakness, tingling, or pain travelling down the arm. Gout can inflame a joint, while fibromyalgia may produce widespread pain and tenderness. Influenza, dengue fever, brucellosis, hypothyroidism, Long COVID, and obesity can be associated with body aches or reduced function in some people. These conditions have different patterns, so shoulder pain and weakness alone cannot confirm one.

Protect the shoulder from heavy lifting and painful overhead movements, but keep gentle movement within a comfortable range. Support the arm when resting and use a wrapped cold pack after a recent injury or a warm pack for stiffness if either helps. Note whether weakness means pain-limited movement or a true inability to lift, rotate, or grip. Arrange an examination when weakness persists, follows an injury, or affects dressing, work, or sleep.

Seek emergency help for sudden arm weakness with facial drooping, speech difficulty, severe headache, chest pain, or breathlessness. Get urgent assessment after a major injury, visible deformity, a cold or pale hand, rapidly worsening weakness, or a hot swollen shoulder with fever. Prompt review is also needed for new numbness that does not settle or pain that is severe at rest.

Ask whether the weakness is caused by pain, tendon damage, nerve involvement, or inflammation, and which movements should be avoided. Ask whether you need imaging, blood tests, physiotherapy, or a specialist review.
