What should you know about sharp pain during sex?

Pain at the vaginal opening may follow dryness, skin inflammation, irritation, or involuntary tightening of the pelvic floor. Pain deeper in the pelvis can occur with conditions such as endometriosis, an ovarian endometrioma, or pelvic inflammatory disease. A single episode after friction may settle, but recurring or worsening pain should be assessed rather than pushed through.

Notice whether the pain starts with penetration, appears with deeper thrusting, or continues afterward. Burning, itching, soreness, or visible skin changes can accompany vulval or vaginal irritation; vulvodynia and lichen sclerosus are examples of conditions that can cause external pain. Dryness related to hormonal change may occur with genitourinary syndrome of menopause. Pelvic pain around periods, bowel movements, or urination can be a presentation of endometriosis. Unusual discharge, bleeding after sex, fever, or lower abdominal tenderness needs medical review because pelvic inflammatory disease can present with these symptoms.

These patterns can guide an examination, but they cannot identify the cause by themselves. Pain is a symptom, not proof of a particular condition.

Stop or change the activity when sharp pain begins. Allow irritated tissue time to settle, and consider a water-based lubricant if dryness or friction may be contributing. Avoid fragranced washes and products applied inside the vagina. Write down where the pain occurs, when it begins, whether it relates to your menstrual cycle, and any discharge, bleeding, urinary symptoms, or skin changes.

Arrange an appointment with a gynaecologist or primary-care clinician if the pain returns, affects intimacy, or does not improve. The assessment may include questions about sexual and menstrual history, an external examination, and a gentle pelvic examination when appropriate. Do not continue penetration to test whether the pain will pass.

Seek urgent medical care for sudden severe pelvic pain, fainting, heavy bleeding, fever with pelvic pain, repeated vomiting, or pain with a positive pregnancy test. These features can signal a problem that needs prompt assessment. Seek timely care for new bleeding after sex, foul-smelling discharge, sores, or pain when urinating, especially if symptoms are increasing.

You could ask: What does the pain pattern suggest? Could dryness, a skin condition, pelvic-floor muscle tension, infection, or endometriosis be involved? Do I need an examination or tests? What symptoms should prompt urgent help? Which sexual activities or treatments are appropriate while the cause is being assessed?
