Sharp ovary pain during ovulation: what to know

Ovulation pain, also called mittelschmerz, may feel like a brief stab, cramp or ache on one side of the lower abdomen. It can happen when an ovarian follicle stretches or releases an egg, and it may switch sides in different cycles. Mild pain that settles without other symptoms is often monitored at home.

Pain that keeps returning, lasts beyond the mid-cycle window or interferes with normal activity can have another explanation. Endometriosis can cause pelvic pain, while an ovarian cyst can produce one-sided pressure or sharper pain. The symptom itself does not identify the cause.

Note when the pain begins, which side it affects, how long it lasts and whether it relates to bleeding, sex, urination or bowel movements. A warm pack over the lower abdomen, rest and gentle movement may help. Use an over-the-counter pain medicine only if it is suitable for you and follow its label directions.

Arrange a routine appointment if episodes are new, becoming stronger, recurring outside the middle of your cycle or affecting work, sleep or sex. A clinician may ask about your cycle and examine you before deciding whether further testing is useful.

Seek urgent medical help for sudden severe pelvic pain, fainting, marked weakness, fever, repeated vomiting, heavy vaginal bleeding or a swollen, rigid abdomen. Seek prompt advice if pregnancy is possible, especially with one-sided pain, bleeding, dizziness or shoulder-tip pain. These features need assessment rather than watchful waiting.
