Bleeding after sex following hysterectomy

The timing of bleeding matters. Sex before the surgical team confirms healing can disturb the vaginal cuff or other healing tissue. Later, friction or dryness may cause surface bleeding, particularly around menopause. The cervix may remain after some hysterectomies, so cervical problems can still matter; cervical cancer presents with abnormal bleeding but bleeding does not establish that diagnosis. Your original condition, such as Endometriosis, Adenomyosis, Endometrial Hyperplasia, Womb Cancer, or Uterine Fibroids, can also affect follow-up needs.

Stop penetrative sex until you have spoken with the surgical or gynaecology team, especially if the operation was recent. Note the amount and colour of blood, pain, discharge, fever, and when sex occurred relative to surgery. Do not insert anything into the vagina while waiting if your postoperative instructions restrict this. A clinician may inspect healing tissue, check for infection, and explain whether further follow-up is needed.

Seek urgent care for heavy or increasing bleeding, severe pelvic pain, faintness, fever, a new foul-smelling discharge, or tissue passing from the vagina. Contact your surgical team promptly for any new bleeding after the recovery period, or for bleeding that returns with sex. Pelvic pain or heavy periods before surgery may also help the clinician understand the wider history.

Ask whether healing is complete, whether your cervix was removed, and when penetrative sex is appropriate for your operation. Ask how dryness, infection, or a vaginal cuff problem would be checked, and whether your original diagnosis changes the follow-up plan.
