Sex after IUD insertion bleeding: what to know

Bleeding after sex soon after IUD insertion can follow cervical irritation, but severe pain, heavy bleeding, fever or a change in the strings needs prompt medical advice. The cervix may be sensitive after insertion, and spotting can overlap with the bleeding pattern caused by a new device. Sex should not be used to test whether the IUD is in place.

Light spotting can occur after an IUD is inserted. Penetration may irritate the cervix or vaginal tissue, particularly when there is dryness or tenderness. Bleeding after sex can also be linked with an infection such as chlamydia, cervical inflammation, cervical dysplasia or, less commonly, a cancer affecting the cervix or vagina. The symptom does not show which explanation applies.

A sudden change in thread length, threads that cannot be felt when they were previously present, or feeling hard plastic may suggest that the device has moved or been expelled. Only an examination can confirm its position.

Use a pad to monitor bleeding and avoid penetration until discomfort has eased. Check the IUD threads only as you were shown, without pulling them. Arrange a review if bleeding after sex continues, if you have a new partner or infection concern, or if the threads feel different. A clinician may examine the cervix, test for infection and check the IUD position. Use backup contraception if displacement is suspected until you receive advice.

Seek urgent care for severe or worsening lower abdominal pain, heavy bleeding, fever, chills, vomiting, faintness, a positive pregnancy test with pain or bleeding, or suspected IUD expulsion. Prompt review is also needed for foul-smelling discharge or pain during sex that is getting worse. These symptoms can indicate infection, pregnancy-related complications or a device-position problem.

Ask when sex can resume, whether your bleeding pattern is expected for your type of IUD, how to check the threads, whether you need infection testing, and what to do if a thread changes. Tell the clinician when the device was inserted and describe the bleeding, pain and discharge precisely.
