Sex after birth bleeding: what to know

Bleeding after sex soon after birth can reflect healing tissues or postpartum bleeding, but new or heavy bleeding needs medical assessment. The vagina, cervix and any stitches need time to recover, and hormonal changes can leave tissues dry or tender. Penetration may irritate healing areas and bring blood to the surface.

Postpartum bleeding can continue for a while after delivery and may change in colour and amount as the uterus heals. Sex can make existing blood more noticeable. Bleeding can also come from a vaginal tear, an episiotomy scar, inflamed tissue, dryness or an infection. A Caesarean birth does not remove the need for vaginal and cervical healing, because postpartum bleeding comes from inside the uterus.

Bleeding after sex is a symptom, not a diagnosis. Cervical dysplasia, chlamydia, cervical cancer or vaginal cancer can present with bleeding after sex, although other explanations are also possible. Persistent or repeated bleeding deserves an examination.

Pause penetrative sex while bleeding or pain continues, and use pads so you can observe the amount and pattern. Do not put tampons or other items in the vagina during postpartum recovery unless a clinician has told you to. When you resume, go slowly, use a water-based lubricant if dryness is present, and stop if pain or bleeding returns. Arrange a postnatal review if bleeding keeps happening; the clinician may inspect stitches, the cervix and the vaginal walls.

Seek urgent medical help for heavy or rapidly increasing bleeding, large clots, faintness, severe pelvic or abdominal pain, fever, chills, or unpleasant-smelling discharge. Contact a maternity or gynaecology service promptly if bleeding after sex is repeated, occurs after postpartum bleeding had settled, or follows worsening pain. These features can signal infection, a problem with healing or another condition requiring examination.

Ask: Could the bleeding be coming from a stitch, tear, cervix or uterus? Is healing progressing as expected? When is penetration appropriate for me? Do I need an examination or infection testing? What amount of bleeding should prompt urgent help?
