Severe Stretch Marks in Pregnancy: What to Know

Short answer
Severe stretch marks during pregnancy are usually a skin response to stretching, but painful or unusual changes should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the marks represent
Stretch marks, also called striae, form when deeper skin layers adapt to rapid expansion and hormonal changes. During pregnancy they often appear on the abdomen, breasts, hips, thighs or lower back. Early marks may be pink, red, purple or darker than surrounding skin and can feel itchy or slightly raised. They commonly become paler and flatter after pregnancy, although texture may remain visible. Their presence does not show that a pregnancy is unhealthy or that skin care has been neglected.
Comfort and treatment planning
Use a fragrance-free moisturiser, avoid scratching, and choose loose breathable clothing. Keeping the skin comfortable may reduce tightness and itch, but creams cannot reliably erase established marks. Discuss any active treatment with your maternity clinician before a procedure or prescription, especially during pregnancy or breastfeeding. After this period, a dermatologist may discuss microneedling, fractional laser treatment, RF microneedling or carboxytherapy, matching the option to skin tone, texture and timing.
When to seek care
Contact your maternity team or doctor if the skin becomes intensely painful, blistered, ulcerated, hot, rapidly swollen or associated with a widespread rash. Severe itch across the body, particularly with other pregnancy symptoms, also needs medical advice because it may have a cause unrelated to stretch marks. Seek review for sudden changes that do not follow areas of stretching.
Questions for your doctor
Ask whether the marks have the typical appearance of striae, which moisturisers are suitable for your current stage, and when procedural treatment could be considered. Clarify expected changes in colour and texture, recovery needs, and how your skin tone affects the choice of treatment.
